Registration Forms
2008 Registration Forms are now Available!
Here at Kelowna Minor Football we have our registration forms available online. Just right click on the name of the form and choose "save target as" or "save link as" and download, print, and fill them out in their entirety, then send the forms in to our league registrar. All Online Forms MUST be accompanied by a photocopy of the Player's Birth Certificate, a copy of the Volunteer Form, and the Medical Form below.
